9d09dca90a MASTER_SITE_KDE 
- switch ftp.uvsq.fr from http to ftp

MASTER_SITE_FREEBSD_LOCAL
- switch se and uk mirrors to http and move upwards in list
- switch ru mirror from http to ftp

MASTER_SITE_PACKETSTORM
- replace all currently listed mirrors with the 4 offical ones

MASTER_SITE_MYSQL
- remove ftp.rhnet.is and mirror.services.wisc.edu
- switch a couple of ftp mirrors to http
- ftp://ftp.linorg.usp.br beomes http://linorg.usp.br
- switch.ch becomes first ftp mirror to improve "make peek-ftp"

MASTER_SITE_ISC
- remove ftp.epix.net
- move ftp.arcane-networks.net below ftp://ftp.isc.org

mirror and zap download2.berlios.de which no longer exists.

2c88c52787 remove all the individual sourceforge mirrors, just rely on the redirector 
at downloads.sourceforge.net; all the FRS mirrors just redirect us back there
to lookup the file as ports don't have logical folder names in the file paths.

add an XXX comment because we don't _really_ want to be relying on this:
to be revisited.

noticed after no-longer-existent mirrors pointed out by fgs@

f18e4d4cba Add a sample file with a number of settings which many porters are 
using in mk.conf, especially a bunch of .poison lines for common typos,
including suggestions from aja, espie, naddy and others. Can be used
with .include in mk.conf.

oks from at least william@ phessler@ and several users like it too.
