SPDX is an international standard for documenting software license
requirements. Remove the existing headers and replace with a brief
SPDX preamble.

The script used to convert the files is added to "tools", and the
file header templates in headers/ are updated.

Additional listing options:
-Ld to display counts in decimal
-Lp to output a list file in every pass (to make sure one exists)
Clean up the help output and make it comprehensive. The -hf and -y
options are no longer necessary, although they are supported for
backwards compatiblity.
Fix macro-levels so it actually count descent levels; a new
macro-tokens limit introduced for the actual token limit.
Slightly simplify the limits code.

"compiler.h" already includes a bunch of common include files. There
is absolutely no reason to duplicate them in individual files, and in
fact it robs us of central control of how these files are used.

with the inline function for mempcpy() we need it there anyway.

Split lib/ into nasmlib/ (for nasm-specific functions) and stdlib/
(for replacements for C library functions which may be missing.)
Rename the ersatz inttypes.h to nasmint.h so we can use a simple test
in compiler.h instead of dealing with include path magic.
Remove tests in configure.in for ancient missing functions (which will
break the build anyway.)

Move ofmt->current_dfmt into a separate global variable. This
should allow us to make ofmt readonly and removes some additional
gratuitious differences between backends.

__OUTPUT_FORMAT__ must consist of shortname of output format
or its alias, otherwise userspace ABI gets broken.
For example source code still can refer to __OUTPUT_FORMAT__=elf,
instead of __OUTPUT_FORMAT__=elf32.

ARRAY_SIZE is a well known name pointing out that
we're dealing with array in macro argument.
Also to be on a safe side prefix_name helper should
check the index been in bounds more precisely.

Remove a bunch of function pointers in the output stage; they are
never changed and don't add any value. Also make "ofile" a global
variable and let the backend use it directly.
All we ever did with these variables were stashing it in locals and
using them as-is anyway for no benefit.
Also change the global error function, nasm_error() into a true
function which invokes a function pointer internally. That lets us
use direct calls to it.

Add new copyright headers to the new output modules. As far as I
know, the only module which we still don't have a green light to
release under 2-BSD is outmacho.

Move backend-specific code into the output/ directory, and make the
null debugging backend a separate file (it certainly isn't needed for
ndisasm...)
